7 questions to ask any AI voice receptionist vendor before signing

Every AI voice agent vendor will tell you their product is bilingual, fast, and easy to set up. The demo will look great, because demos are built to. The way you protect your clinic is not by trusting the pitch, it is by asking the same hard questions of every vendor and seeing who can answer them plainly and who gets vague. This is a vetting checklist, not a comparison, and it works no matter which vendor you are talking to.

Here are seven questions to ask before you sign, why each one matters for a bilingual, cross-border clinic, and what a straight answer sounds like.

1. Does it speak real English, or is it machine translation? Prove it.

This is the first question because it is the one most likely to be fudged. Plenty of tools are a Spanish bot with a translation layer, and they produce the stiff, off English a San Diego or Phoenix patient spots in one line. Ask the vendor to prove it, not describe it. Can they send you real transcripts of English conversations with actual patients? Was the English tested with U.S. patients, or just generated? A confident vendor hands over transcripts without flinching. If they can only talk about it in the abstract, assume it is translated.

2. What happens when it does not know the answer?

A voice agent that guesses is worse than no agent at all, because a made-up price or a wrong medical answer costs you trust and can create real liability. Ask exactly what happens at the edge of the script. Does it escalate to your team, and how fast? Does it ever invent a price, a diagnosis, or a promise to cover something? The answer you want is clear: it hands off cleanly to a human in seconds for emergencies, complaints, and anything off-script, and it never fabricates an answer to fill a gap. If a vendor cannot describe the escalation path in one sentence, it probably does not have a good one.

3. Where does patient data live, and does it meet Mexican data law?

If your clinic operates in Mexico, patient data handling is not optional and it is not a formality. Ask where the data is stored, who can access it, and whether the vendor complies with the LFPDPPP, Mexico's federal data protection law. Do they capture patient consent inside the conversation? Is there a published privacy notice? Are the logs auditable? A vendor built for this market answers all of that without hedging. One that treats Mexican data law as an afterthought is a risk you are signing up to carry.

4. Does it actually book into my calendar, or just collect a lead?

There is a big difference between an agent that books an appointment and one that just gathers a name and number for someone to follow up on tomorrow. The second kind is barely automation. Ask which calendar and practice systems it integrates with, whether it writes the appointment directly into your calendar, and whether it checks real availability before it offers a time. Google Calendar, Outlook, and Open Dental are the ones a clinic usually needs. If the tool cannot close the loop into your actual schedule, the after-hours booking you were promised is really just a message in a queue.

5. Which channels does it cover, and does it keep my current number?

Your patients reach out on WhatsApp, by phone, and from your website, so ask whether one agent covers all of those or whether you are buying a website widget that only works in one box. Ask specifically whether it uses the official WhatsApp Business API and whether you keep your existing clinic number, because a setup that forces a new number breaks the continuity your patients rely on. The answer you want is that it runs on the channels your patients already use, keeps your current number through the official API, and behaves consistently across all of them.

6. How long does setup take, and what do you need from me?

Ask for a real timeline, not "quick." How long from signing to the agent answering live patients? What exactly do they need from you, and how much of your time does it cost? A vendor that knows its process gives you specifics: your number connected in about 48 hours, a training session where you hand over prices, services, doctors, and FAQs so the agent speaks in your clinic's voice, and full production within about a week. A vague answer here usually means a long, messy onboarding you will be babysitting.

7. Is the pricing clear, and can I leave when I want?

Finally, read the deal, not just the monthly number. Ask what the price includes, what is an add-on, whether there are long-term contracts, and what happens to your data if you cancel. Promotions, insurance verification, and list reactivation are often optional add-ons, and that is fine as long as it is stated plainly. What you want to hear: transparent pricing, no long-term lock-in, and a clean exit where you keep your number and get your conversations and patient data exported if you leave. A vendor confident in the product does not need to trap you in the contract.
